WAIVER:

  1. Acknowledge that camp participation does not include insurance coverage of any kind.

  2. Release and forever discharge the Durham GFC, its authorized agents and representatives from any and all causes of action, claims, damages, loss or injuries of every nature and kind, howsoever rising from, or may hereafter have as a result of participation in this program.

  3. Authorize Durham GFC to use photos of participant to appear on brochures, websites or media coverage.
